NOTE: IF YOU SEND US A CHECK BUT NO FORM, WE WILL SEND THE CHECK BACK TO YOU. WE HAVE NO WAY OF KNOWING WHO YOU ARE, WHICH ACCOUNT TO APPLY THE PAYMENT TO UNLESS YOU SEND THE FORM WITH YOUR CHECK!
